Student Forms

• Accommodation Request Form (to request student accommodations)
• DSO Testing Form is located on Accommodate for all students registered to use the DSO Testing Center
• DSO Complaint Form (for concerns about access and/or accommodations)

Faculty and Staff Forms

• Submit Test to DSO Testing Center (via email); can also be completed via Accommodate
• KU Employee Reasonable Accommodation Form to request reasonable accommodations. If you cannot access this form or need it in an alternate format, please email DSO@kutztown.edu.
